Terms and Conditions
For Healthcare Providers booking Live-in Nurses for patients in their own homes
The following notes are intended to summarise important extracts from our Terms of Business. They are not the Terms themselves. A full copy of our terms and conditions of business can be requested from the Consultus Head Office.
Consultus is a Registered Nursing Agency specializing in the supply of qualified Nurses into live-in positions.
Our Nursing Manager, who is a Registered Nurse, will discuss with you your requirements and suggest an anticipated date for the start of service. If you are in agreement with these terms, she will liaise with and, with your consent, assign appropriate Nurses. We will advise you of the names of any Nurses who have agreed to attend for the advised period (or for the first weeks of service if care is to be ongoing). A cancellation fee may now be payable in accordance with our full terms of business.
We shall inform you of the name of a member of staff at the agency who is responsible for the supply of workers to you. You may, in the first instance, contact this person (or the out-of-hours on-call service) if you need to speak to a booked Nurse.
If you receive details of a Nurse from us and you have received the same details from another Nursing Agency within the last six months please notify us immediately.
No later than 24 hours after the commencement of your booking, a Nurse, usually the first booked Live-in Nurse, will (with your participation if appropriate) carry out a service user care needs assessment and risk assessment, and mutually agree an appropriate plan of care. The Care Plan will be put in writing for the Nurse and her successors to follow. You or your authorised representative should sign this. The Nurse will send a copy of the Assessments and Care Plan to our office where the manager will keep it on file. If you would prefer us to use a Care Plan and Risk Assessment that has been prepared by you or someone else please discuss this with the Manager.
At regular intervals during the course of the assignment(s), a Nurse Consultant will contact you. She will conduct a review of the work being carried out by the Nurse and ascertain that it is being done in accordance with your requirements, our policies and best nursing practice.
In the unlikely event our Nurse is unable to continue work due to sickness or other reason we will endeavour to have a substitute in place before this Nurse leaves. However, before commencement of service, please notify us of any preference you have for urgent interim care, e.g. local sub-contractor agencies; alternatively you may ask us to arrange a nurse-on-retainer (separate terms on request).
Consultus live in Nurses are 'agency workers' who are supervised and paid by us. Consultus acts as a 'provider of care'. Our invoice covers the entire cost of the service, including Nurses' wages and all statutory contributions. The patient or a responsible person must sign the Nurse's timesheet at the end of the week; this confirms the Nurse's working hours and satisfaction with the service.
Clients can pay invoices by Direct Debit, for which a discount will be awarded (Live in Care Service only), by cheque or by most debit and credit cards.
