Admission Avoidance & Early Discharge from Hospital

Primary Care Trusts (PCTs) are under great pressure to reduce costs.  One way of doing this is to look at the number of people admitted into hospital.

Hospital facilities are expensive and a hospital stay is usually the most costly episode in the patient’s experience of care.  Managers and clinicians who commission or run emergency, intermediate and community services, together with long term continuing care, are therefore under pressure to improve management of this group and to source cost efficient alternatives.

By using Consultus’ live in nursing service for a range of conditions, we believe that we can generate gross savings for our PCT or Health Board (HB) Clients by working closely with:

  • GPs (General Practitioners), especially those providing Accident & Emergency triage, to identify patients who could be prevented from entering hospital. This would offer savings against the whole cost of the admission or at least convert a full admission into a short term tariff admission

  • Hospital and community staff to facilitate early discharge from hospital. This would save the our PCT or HB Clients having to pay the excess day rate over the trim point or by reducing the length of stay after medical stabilisation

  • District (Community) Nurses to provide intensive nursing care to people with long term chronic nursing conditions. This would offer considerable savings against the cost of hospital re-admissions and reduce the increasing workload of District Nurses

  • Continuing Care teams to avoid respite or palliative care being undertaken in hospital, nursing home or hospice where institutional care is inappropriate
