End of Life Nursing

In June 2008 the Department of Health published their End of Life Care Strategy promoting high quality care for all adults at the end of life.  Healthcare commissioners were tasked with:

  • Identifying patients in need of palliative care
  • Assessing their needs, symptoms and preferences
  • Planning care around the patient’s needs and preferences, particularly allowing patients to live and die where they choose

Although every individual may have a different idea about palliative care, most would value being:

  • Treated as an individual, with dignity and respect
  • Without pain and other symptoms
  • In familiar surroundings
  • In the company of close family and friends

Consultus' Care and Nursing teams have helped thousands of people to achieve their wish of dying in the privacy of their own homes.

As part of our renown 24/7 home nursing service, Consultus Nurses provide:

  • A personalised care plan that will take into account their Client's needs, preferences and choices
  • Their Client with relief from pain and other distressing symptoms
  • Continuity of care with two or three Nurses on a rota
  • Social, psychological and spiritual support
  • A source of expertise for other healthcare professionals to ensure the best support systems are put in place
  • Assistance to allow the Client to lead as active a life as possible
  • Help and advice for the Client’s family during the Client's illness and following their bereavement
