What nursing services can you provide?
Our Continuing Healthcare service enables PCTs to transfer the management and delivery of healthcare packages with varying levels of complexity to the homecare environment. Our Live in Nurses are available to look after individual patients or couples in short or long-term bookings where continuity of care is required. This includes: · End of life/Palliative nursing care · Hospital admission avoidance or care to facilitate earlier discharge · Chronic medical illnesses such as Multiple Sclerosis, Motor Neurone Disease, ‘Strokes’ or Parkinson’s Disease · Rehabilitation for people who need nursing care following surgery, a period of illness, or hospitalisation · Respite care Dementia and Alzheimer’s Disease · Patients who would benefit from an alternative to residential nursing home care
